Pain Management & Symptom Control
We at City Hospice believe that no one needs to die in pain, we offer a form of palliative care that is intended to relieve pain, maintain comfort and symptom management once curative treatment has ended.

Medication Management
Our doctors approve medication orders in a timely manner to ensure patients get their medications when needed. The Nurses may assist patients with pill box refill and educate the patient and family members on how and when patients should take their medications.

Medical Equipment & Supplies
The medical equipment company we work with ensures that equipment and medical supplies are sent out fast once we place an order.

Physician Services
Our doctors ensure that our patients care wishes are honored and their care goals are achieved. They respond very quick when needed to give orders.

Nursing Visits
City Hospice nurses are available to monitor the patient’s well-being, make the patients comfortable and manage their symptoms. Our nurses pay visit to the patients anywhere they call home. The number of visits per week is determined by the care goals of the patient.

Home Aide Visits
Our certified nursing assistants visits the patient to bath and care for the patients as scheduled.

Counseling and Spiritual Support:
We offer spiritual support, emotional support, bereavement support and counseling through our Chaplain. We will be there all the way with our patient and family members to ensure that end of life is comfortable.
